Multidisciplinary Care - All Tumour Groups

Update on SMICS' activities related to multidisciplinary team meetings

The Continuum of Care Project (CCP) commenced in 2007, with an initial review of multidisciplinary teams across the SMICS catchment (phase 1). You will find more information about Phase 1 here.

Since that time, SMICS has undertaken two further phases:

Phase 2: consumer consultation and health professional survey
Phase 3: support and development of multidisciplinary team meetings

Continuum of Care - Phase 3

Phase 3 has focused on supporting multidisciplinary team meetings across the catchment. SMICS has funded project officer roles at Alfred Health, Peninsula Health and Southern Health until December 2010 to help support and develop the meetings with the likes of:

• meeting terms of reference and agendas
• referral protocols and forms
• medical record forms
• expanding the membership to include allied health and nursing (where required)
