SMICS is piloting an innovative model of care at Cabrini Health

In December 2008, SMICS commenced a collaborative project with Cabrini Health to develop a model of colorectal care coordination.

The process has involved an initial scoping phase, with an agreed model of care now being piloted. The project included the recruitment of a Colorectal Nurse Coordinator, whose role is to oversee the pilot of the model over 18 months.
