SMICS is piloting an innovative model of care for haematology patients across the catchment

Haematological cancer care is complex and poses specific challenges for health professionals. Malignant haematology services are provided by Alfred Health, Cabrini Health, Peninsula Health and Southern Health.

Through the development of comprehensive data collection tools and a robust evaluation framework, a Model of Malignant Haematology Care Coordination has been developed and will be piloted over 18 months.

SMICS appointed a Malignant Haematology Care Nurse Coordinator, with the role being based at Alfred Health. The role is responsible for the design and implementation of the model across the SMICS catchment. In addition, the development of a survivorship program will commence in 2010 to address the complex needs of these patients.
